What is a DOT/SAP Evaluation, and when is the evaluation required?

When you, the employee, violate DOT drug and alcohol regulations, you cannot again perform any DOT safety-sensitive duties for any employer until you complete the SAP evaluation, referral, and education/treatment process that is outlined in the applicable DOT agency regulations.

The first step of the SAP process is to complete an evaluation.

The Substance Abuse Professional (SAP) is a person who evaluates employees who have violated a DOT drug and alcohol program regulation and makes recommendations concerning education and/or treatment, follow-up tests, and aftercare.

 

The SAP is not an advocate for the employer or the employee. The SAP function is to protect the public interest in safety by professionally evaluating the employee and making appropriate recommendations.

The SAP Evaluation Process Involves:

  • Initial face-to-face clinical evaluation including standardized testing which lasts approximately two to two-and-a-half-hour
  • Recommendation for treatment and/or education is made in every case as required by DOT rules.
  • The initial evaluation report is then sent to your employer.
  • The SAP assists you in reaching out to the treatment provider and arranging for its services.
  • The SAP maintains contact with the treatment provider.
  • The SAP will conduct a face-to-face and a follow-up clinical evaluation.
  • The SAP informs your employer of your compliance or non-compliance with the SAP’s recommendations.
  • Your employer determines whether or not you can return to work.
  • The SAP will send a follow-up report and follow-up testing plan to your employer.
  • The SAP may also include an aftercare plan which should be followed.

The above information is retained for your future employer if you have been released from your employment.

If you or the employer requests for an evaluation, please provide the following at your initial assessment:

  1. Your Name, Title, Company Name, Address, And Phone Number
  2. Type of DOT Modality, i.e., FMCSA, FRA, FTA, FAA, RSPA, or USCG
  3. Your Date of Birth and Social Security Number
  4. Type and Date of Violation
  5. Current Employment Status
  6. The Name and Phone Number of Your Designated Employer Representative (DER)
  7. Who Will Be Responsible for Paying for The Evaluation

If you are the employer and are referring your employee for evaluation, please provide the following at the time of your call:

  1. Your Name, Title, Company Name, Company Address, And Phone Number
  2. Type of DOT Modality, i.e., FMCSA, FRA, FTA, FAA, RSPA, or USCG
  3. Employee Name, Date of Birth, Social Security Number, Address, And Phone Number
  4. Type and Date of Violation
  5. Current Employment Status of the Employee
  6. The Name and Phone Number of Your DER (if different)
  7. Who Will Be Responsible for Paying the Evaluation (Employer/ Employee)
